The information we collect
(a) Information you provide directly. When you contact us, schedule an appointment, or become a patient, we collect information such as your name, address, phone number, email address, date of birth, insurance information, and health information relevant to your care.
(b) Information collected automatically. Like most websites, our site uses cookies and similar technology to operate properly and understand how visitors use our site. Details on what cookies we use and your choices around them are in our Cookies Policy below.
The term "Information" in this policy refers to both types described above, as well as any information you provide to us directly by phone, text/SMS message, mail, or any other written or oral communication with us.

We use your information to schedule and manage your care, communicate with you about your appointments, process billing and insurance claims, and administer our normal business operations. We do not sell, rent, or share your personal information — including your name, phone number, or SMS opt-in status — with third parties or affiliates for marketing or promotional purposes. This information is used solely to send you appointment-related text messages. You may opt out at any time by replying STOP, or request help by replying HELP. Message and data rates may apply; message frequency will vary.

By opting in to receive SMS messages from Total Motion Sports Therapy, you agree to receive appointment reminder messages at the mobile number provided. Message frequency varies. Changes To This Policy
We may revise this Privacy Policy from time to time. Revisions take effect when posted. Your continued use of our services following any changes constitutes your acceptance of those changes.
Obtaining SMS Consent
Consent to receive SMS appointment reminders is obtained in one of two ways:
-
By phone, at the time of scheduling. When a patient calls to book an appointment, our front-desk staff verbally informs the patient about our text message reminder service and asks for permission to send reminders to the mobile number provided. The patient may accept or decline at that time.
-
In person, at the patient's first visit. During new-patient intake, patients review and sign a written consent form. That form includes the following language:
"I agree to receive SMS appointment reminders from Total Motion Sports Therapy. Message and data rates may apply. Message frequency varies. Reply STOP to opt out at any time, or HELP for help. Consent is not a condition of receiving care."
A patient may opt in, decline, or opt out of text reminders at any time without affecting their ability to receive treatment at our clinic.
What Patients Receive
Once opted in, a patient receives one SMS reminder per scheduled appointment, sent approximately 24 hours in advance (or shortly after booking for same-day appointments). Each message includes the appointment date, time, and a request to reply YES to confirm or NO to cancel or reschedule. We do not send promotional or marketing text messages of any kind.
Message Frequency, Rates, and Opt-Out
-
Message frequency: varies based on how many appointments you have scheduled.
-
Message and data rates may apply, depending on your mobile carrier and plan.
-
To opt out at any time, reply STOP to any message. You will receive a one-time confirmation that you've been unsubscribed.
-
For help, reply HELP, or contact us directly (see below).
-
Consent is not a condition of receiving care, purchasing services, or scheduling an appointment.
Your Information Is Never Shared for Marketing
Your mobile phone number and SMS opt-in status are used solely to send you appointment-related text messages from Total Motion Sports Therapy. We do not sell, rent, or share this information with third parties or affiliates for marketing or promotional purposes.
For more detail, see our Privacy Policy and Terms of Service.
